Echocardiography is an important tool for risk stratification including the determination of LA size, spontaneous contrast, LA appendage flow velocity and LV dilatation. Idiopathic paroxysmal fibrillation in younger patients has a lower risk and can be treated with aspirin. Before cardioversion, of AF of > 48h duration, a 3 weeks anticoagulation period is mandatory, and prolonged for at least one month after successful therapy. For AF of shorter duration, transoesophageal echocardiogram can verify the absence of intra atrial thrombus, and allow cardioversion under heparin therapy, followed by the same oral anticoagulant therapy for at least one month. International recommendations have been edicted, but are not fully understood and followed in real life, as demonstrated by several surveys.
